Oska Health, a hybrid healthcare provider combining human coaching with artificial intelligence, has secured €11 million in seed funding to expand its continuous care model for patients with chronic conditions.

The round was led by Capricorn Partners and SwissHealth Ventures, with participation from Revent, Calm Storm, LBBW Venture Capital, BMH, GoHub Ventures, and Aurum Impact.

Addressing Gaps Between Doctor Visits

Chronic diseases remain one of the largest pressures on healthcare systems worldwide. While clinical treatment pathways are often well defined, patients frequently struggle to maintain medication adherence and long-term lifestyle changes between appointments.

Research indicates that a significant portion of chronically ill patients do not consistently follow prescribed treatments, increasing the risk of complications and avoidable hospitalizations.

Oska Health focuses on multimorbid patients living with multiple long-term conditions such as chronic kidney disease, diabetes, and hypertension. Its model is designed to provide structured support in the periods between physician visits.

Blending Human Coaching With AI

The company delivers personalized care through a combination of certified health coaches and AI-driven tools. Patients receive ongoing support via video consultations, messaging, and a digital therapy application.

Coaches guide individuals on nutrition, physical activity, medication routines, and health literacy, while the AI infrastructure helps streamline administrative tasks, personalize recommendations, and scale care delivery.

According to CEO and co-founder Niklas Best, effective chronic disease management depends largely on daily habits rather than occasional clinical interventions. He noted that combining human interaction with AI allows Oska Health to deliver scalable, high-quality support while reducing operational complexity.

Expanding AI Care Infrastructure

With the new capital, Oska Health plans to further develop its AI-powered care platform and expand its reach across healthcare systems. The company has been supporting patients with chronic conditions for more than three years and aims to strengthen its position in the growing digital chronic care market.
